Hello and welcome to our practice.  Our group was founded to evaluate and care for patients with cancer and blood disorders. We have a competent, professional, and compassionate staff that is committed to making your experience with us as positive as possible.

We take pride in delivering high-quality medical care in an upbeat and supportive environment. Treatment plans are tailored to each individual and their unique circumstance.

All care is coordinated with the patient's primary care, referring, and other specialist physicians. We have a number of physicians allowing us to offer a wide range of personalities, experience, and expertise to suit each patient's needs.

All of our physicians are board certified in Internal Medicine and Medical Oncology and/or Hematology. For more information on our physicians, see their individual biographies.

As a new patient you will be contacted by our New Patient Coordinator. Your insurance information will be verified and our participation with your insurance company will be confirmed. We participate with most major insurance companies and will submit claims on your behalf. We do not accept Workman’s Compensation.

You will be asked to complete a patient information form prior to your appointment. Please be sure to have your valid insurance and personal identification cards with you at the time of your appointment. A photocopy will be placed in your billing file. Please be assured that your confidentiality will be respected at all times.

If you are covered by more than one insurance company, please let us know which company is the primary carrier. Failure to do so could delay payment of your insurance claim. If there are any changes to your insurance coverage at any time, please contact our office so that we may update your records.

Medicare patients without a supplemental insurance policy will be responsible for 20% coinsurance, payable at the time of service.

Copayments are payable at the time of your visit. We are unable to bill you for your copayment.

We accept cash, check, Visa, and Mastercard.

If you have any questions regarding our financial policies, please phone our Patient Accounts Representative.

Our front desk receptionists will help you with scheduling appointments and/or tests. Your next appointment needs to be scheduled before you leave the office. To ensure safety and efficiency, you must have an appointment for each office visit, whether it is to see a doctor or physician's assistant, for lab work only, for a shot or for chemotherapy.

For your convenience we are providing our New Patient Registration Form to download. You can use Microsoft Word to open, complete, and print the form.
